Outpatient Rehabilitation FAQs

What do I need to bring the first day?
When you come in for your first appointment, please bring your physician referral if you have one, a valid Driver’s License or ID, your insurance card, a list of your current medication list and allergies, and any other additional records you may want us to see.
When should I arrive for my appointment?
We recommend you arrive at least 15 minutes prior to your initial evaluation to complete the registration and medical history intake forms. We recommend you arrive at least 5 minutes early to each follow up appointment.
What clothing should I wear to my appointment?
Wear comfortable clothing or active wear and avoid open-toed shoes if you receive Physical Therapy.
How long are appointments typically?
Our appointments typically last 30 minutes to one hour.
How does the referral process work?
In order to receive therapy, we require an order from your physician. We take orders or scripts by fax, walk-ins, and through Epic, our medical record system. Once we receive your information, we will set you up with the first available appointment and will verify your insurance as a courtesy before your first follow-up appointment. You will be evaluated by a Physical Therapist, Occupational Therapist, or a Speech-language Pathologist. Depending on your needs, you may be evaluated by more than one discipline.
What insurances do you accept?
We accept most insurances including Medicare, Medicaid, most private insurances, and Workers’ Compensation. We verify your insurance information as a courtesy.
How much will therapy cost?

Each insurance company is different and most have multiple plans. We will verify your insurance as a courtesy prior to your first follow -up appointment and will inform you if your insurance requires a co-payment, co-insurance, or if you are out-of-network. The majority of our patients do not have out-of-pocket costs for therapy, but some have co-pays or co-insurance based on the plan. If you have additional questions on potential cost, please feel free to reach out to us or review your insurance coverage policies.

How many visits will my insurance cover?
Each plan differs, but the amount of visits that you attend are determined through a combination of your physician’s order, the Plan of Care completed by your therapist, your insurance coverage, and the progress you make while in therapy. It is difficult to determine until after your initial appointment, and it is reassessed throughout your episode of care.
Do you have translation services available?
Yes, we have translation services available either in person or via a telephone/video medical interpretation service. In some locations, we have Qualified Bilingual Staff (QBS).
